Say the useful part first. If the balance you hold is dollars and the account you are paying is a US dollar account, no currency conversion takes place anywhere in the transfer. That is worth holding on to, because it is not how a dinar to dollar transaction usually works in Kuwait. An exchange company converting dinar into dollars prices that conversion into the rate at the counter, and the rate is where the cost sits rather than in the receipt. If your income already arrives to you in dollars, you can skip that step rather than pay for it.
The point is sharpened by the dinar itself, the highest valued currency unit in the world. A margin that looks trivial when quoted against a single dinar is a substantial number of dollars on a fee paid every month for the length of a contract. The way to avoid arguing about it is to have no conversion in the transfer at all.

Why no rate is fixed on this route

A rate is fixed at confirmation when a conversion happens. On a dollar to dollar transfer there is no conversion, so there is nothing to fix and nothing that can move between confirmation and settlement. The dollars you send are the dollars that arrive.
Where it does apply is the other case. Sending to a US account from a pound or euro balance converts once into dollars, and that rate is shown before you agree and held from that point, exactly as on any other corridor.
For a fixed obligation such as a mortgage payment or a tuition instalment, the dollar to dollar case is the reason people keep the balance in dollars in the first place. The figure you owe and the figure you send are the same figure, month after month.
